*This form is for Group registration only. Museum hours are Monday - Friday 10:00 AM - 3:00 PM. Group Tours can be requested Monday - Friday at 10:00 AM and 2:00 PM. REQUESTED TOURS MUST BE CONFIRMED VIA EMAIL BY THE GUEST SERVICES DESK. DO NOT FILL OUT IF YOU ARE AN INDIVIDUAL.
